In Preparation for your appointment:
  • Please be sure to bring your referral form from your physician with you.
  • Bring your health insurance information, forms and cards with you. Please read/print the patient pre-registration form(s).(pdf)
  • Bring your driver’s license / photo Identification with you.
  • If you have previous X-rays, examination films or reports, bring them with you.
  • If there is a possibility of pregnancy, please inform our staff prior to your appointment.
  • Children 11 and under cannot be left unattended or go into the exam room with you.
  • Patients who are improperly prepared for their exam may need to be rescheduled. Please check prep instructions.(pdf)
  • If you need to reschedule your appointment, please call us at least 24 hours in advance.
  • Do not wear excessive make-up, bobby pins or other metal hair accessories.
  • Please let us know if you have any implanted medical devices.
  • Please leave valuables at home. RAS cannot be held responsible for lost belongings.

Frequently Asked Questions

Q: I have been here before do you still need my insurance card?
A: Yes, the copy we take goes down to our billing office. We take a new copy to ensure accuracy, since employers change as well as insurance companies change.
Q: Do I need a referral form?
A: Yes, we always need a referral form, it is your prescription for having the x-ray.
Q: Is there a prep for my X-ray?
A: Most plain x-rays such as an Chest, Abdomen, Ankle, and other extremities do not need appointments and do not require preps. Some of the more technical x-rays such as a Barium Enema, Pelvic Ultrasound, and Mammograms have preparations.
